Agency For Healthcare Administration (AHCA) Level 2 Background Check in Jacksonville, FL
If you’re applying for or renewing your license under the Agency for Health Care Administration (AHCA), or you work for an AHCA-regulated facility, you’ll need a Level 2 background check submitted through the AHCA Clearinghouse.

Steps to Complete Your Clearinghouse Screening
1) Determine Who Initiates the Screening: If you’re employed by a larger company or healthcare organization, your HR or compliance department will usually initiate your background screening in the AHCA Clearinghouse. Once they do, you’ll receive:

A Live Scan Request Form (with your name and ORI number pre-filled)

A copy of the AHCA Clearinghouse Privacy Policy
2) Schedule your appointment: Bring the required identification documents to your appointment.
3) Fingerprinting Session: Print your pre-populated Live Scan Request form from CHAI or have it available on your phone at the time of your appointment. We will issue you a TCN (Transaction Control Number) at the time of your appointment. Be sure to keep it in a safe place.
4) Track Your Submission: Enter your TCN at the FDLE TCN Lookup to monitor your results.
Failure to register in the CHAI system can result in delays in licensing or employment. We are not responsible for you getting registered in CHAI.
