Department of Education – Vocational Rehabilitation (DOEVR) Level 2 Background Check in Jacksonville, FL
If you’re employed by or volunteering with Florida’s Division of Vocational Rehabilitation (VR), you must complete a Level 2 background check through the AHCA Clearinghouse.
Fingerprinting Jacksonville provides fast, same-day Live Scan submissions to FDLE and FBI, ensuring you meet VR screening requirements and avoid delays in starting work or services.
Please note: Clearinghouse screenings require a photograph to be taken during your appointment.

Steps to Complete Your Clearinghouse Screening
1) Register in the CHAI system: Create or upate your clearingouse profile. Click here to visit The Clearing House Applicant Initiated (CHAI) here.
2) Schedule your appointment: Bring the required identification documents to your appointment.
3) Fingerprinting Session: Print your pre-populated Live Scan Request form from CHAI or have it available on your phone at the time of your appointment. We will issue you a TCN (Transaction Control Number) at the time of your appointment. Be sure to keep it in a safe place.
4) Track Your Submission: Enter your TCN at the FDLE TCN Lookup to monitor your results.
Failure to register in the CHAI system can result in delays in licensing or employment. We are not responsible for you getting registered in CHAI.
