Department of Elder Affairs Level 2 Background Check in Jacksonville, FL
If you work with seniors or provide services through a DOEA-funded or licensed program, you’re required to complete a Level 2 background check through the Florida Clearinghouse.

Steps to Complete Your Clearinghouse Screening
1) Register in the CHAI system: Create or upate your clearingouse profile. Click here to visit The Clearing House Applicant Initiated (CHAI) here.
2) Schedule your appointment: Bring the required identification documents to your appointment.
3) Fingerprinting Session: Print your pre-populated Live Scan Request form from CHAI or have it available on your phone at the time of your appointment. We will issue you a TCN (Transaction Control Number) at the time of your appointment. Be sure to keep it in a safe place.
4) Track Your Submission: Enter your TCN at the FDLE TCN Lookup to monitor your results.
Failure to register in the CHAI system can result in delays in licensing or employment. We are not responsible for you getting registered in CHAI.
